Insurance Plans

We participare with Vision Service Plan (VSP), Davis Vision, EyeMed (not all groups), Medical Eye Services, Mitsubishi Cement, UFCW, and others. Check with our staff if you have other types of coverage.

If you have a medical problem with your eyes, such as cataracts, dry eyes or an infection, you may be able to use your medical insurance to cover your exam. If your plan is PPO or POS, we can give you a bill to submit to your insurance company for reimbursement. We are not part of a medical group, so we cannot bill HMO insurance. If you have HMO coverage, we can inform your primary care doctor of your eye problem, and they can refer you to the eye doctor in your medical group.

Verification of Benefits

For our office to verify your coverage, we will need the following: 

  1. Group, plan or policy number
  2. ID of the insured
  3. Member and patient’s date of birth
  4. Name of employer.

Verification of insurance benefits must be verified 24 hours prior to your appointment. 

If you are uncertain if you have vision coverage, please check with your human resource department or call your insurance company directly and ask the following:

  1. Does my insurance plan provide routine vision benefits and coverage?
  2. What is the name of my vision insurance? (it might be different than your medical insurance)
  3. Can I see any eye doctor of my choice?
